RPN ROLE IN LONG TERM CARE

I will be starting an RPN position at a long term care setting in a week or so
up to this point I have only had experiance working in acute care settings where i worked independently to provide care to my patients. I am wondering if there are any RPN's out there who have had experiance in a long term care setting and can give me some advice or the similarities and differences.

    I work at Fort Vancouver Convalscent Center in Vancouver, wash. I work on a rehab hall; where pt’s are recovering and participating with OT/PT/SP.
    My advice is don’t assume that these pt’s are stable! Good Assessment skills are essential!!
    I have seen it all… everyone pt that I sent back to ED ( I was right on) had Excaberation of CHF, Pneumonia, DVT, PE, Sepis or infection that requires hospitalization, MI, CVA>

    I hope that this helps. I have 10-13 pts.
